Program Information:

Shamanic Convergence 2012 programs contain activities that can be physically and emotionally demanding. We will try to accommodate people regardless of physical disability whenever possible, but we must have full discloser of all physical and mental conditions prior to the Convergence so we can prepare and provide a safe environment for you and for all participating. We require that anyone under the care of a health professional for a current condition consult her/him to make sure these programs and activities, (including sweat lodge) are advisable. All participants must consult and follow the advice of their physicians on taking prescribed medications for the duration of the program.

Privacy:
All information on these pages is solely for the use of Shamanic Convergence 2011. All information will be held in strict confidence, and to the extent of the law will not be released to anyone without your prior consent.

Policy for Children:
I understand that I am responsible at all times for any children under the age of 18 that I bring to the Shamanic Convergence.

Cancellation Policy:
No refunds will be available after September 8, 2011.

Recording and Photos:
The use of video/audio recorders is prohibited unless prior permission is granted. Photographs may be discreetly taken by members of the Shamanic Grandmothers Council of NS and used solely for the purposes of the Convergence for web site, marketing or advertizing purposes.

Participant Release of Liability:

I affirm that the confidential information I have provided is accurate and complete. I understand that failure to disclose this information could affect my own safety and the safety of those around me, and I agree to hold The Shamanic Grandmothers Council of Nova Scotia harmless if full disclosure of a pre-existing medical condition has not been provided. In the event of illness or injury, consent is hereby given to provide emergency medical care, hospitalization or other treatment, which may become necessary.

I understand that parts of the Shamanic Convergence 2012 program may be physically and emotionally demanding. I agree to accept full responsibility and assume all risks, including those caused by acts of God, injury, death, and/or loss to my person and or property knowingly and voluntarily, realizing that The Shamanic Grandmothers Council of Nova Scotia will take all reasonable precautions to minimize these risks. I knowingly, voluntarily and irrevocably waive any and all past, present, and/or future injuries, death, or loss, including those caused by acts of God, received while participating in activities conducted by the Shamanic Grandmothers Council of Nova Scotia, as a participant, spectator and/or visitor, or in any other manner of form, taking part in the exercises, practices, excursions, and/or demonstrations. I certify that I am physically, mentally and emotionally capable to participate in the Shamanic Convergence 2012 despite some of the rigors inherent in such an undertaking.

I understand that activities and dates may change without notice and that the Shamanic Grandmothers Council of Nova Scotia is not responsible for any non-refundable airfare at any time.

I agree to assume all risks and hazards incidental to participating in activities on the lands of Dorje Denma Ling and I hereby waive, release, absolve, indemnify and agree to hold harmless The Shamanic Grandmothers Council of Nova Scotia, their directors, officers, agents and volunteers for any claim arising out of self and others.

My registration for the Shamanic Convergence 2012 indicates my acceptance of these terms and my desire to participate in the Shamanic Convergence 2012 program.
